Unfortunately, if you don't already know how to design a PCB then it's either going to take you a while, or it's going to be expensive.
In terms of designing the PCB, KiCAD is free and unencumbered. It is a most excellent PCB design package.
For fabrication, there are several online services such as JLCPCB and PCBWAY where you can upload the files from KiCAD and have your PCB fabricated very cheaply. They will do a quick engineering check to ensure that the board can actually be made, but you will get exactly what you asked for, so you need to be sure it's exactly what you want. It's cheap to do, so you can iterate your design if you made a mistake.
If you don't have these skills, or the time to learn them then you could pay a designer to do it for you. A board such as what you have described could probably be drafted in a couple of days. Even so, that's going to be a few hundred dollars.
