Update on this:
Added a pullup to QSPI_SS. The only difference between my board and the outlined one (that I can tell) is I used a flash with a different package, same specs (W25Q128JVPIQ TR). Instability is gone now, but the regulator is still at 1.4V.
Holding the RUN button brings the regulator down to 1.1V. Letting go it briefly goes to 1.2V and then back up to 1.4V...
