There's 4.03 seconds between undervoltage being detected and rectified, not microseconds.
vc4-kms-v3d will be reinitialising the display stack to its liking. It is also fussier (but also more flexible) over the display having a valid EDID and behaving correctly with hotplug detect (the firmware drivers don't support hotplug at all).
With vc4-kms-v3d enabled, I'd check the state it believe the display subsystem is in via "kmsprint" to tell you the current mode, and "kmsprint -m" to tell you the modes it believes each display supports. If it reports disconnected, then you've likely got an issue with the hotplug detect line of the HDMI connector or cable.
