A schematic circuit diagram has nothing to do with physical layout, it's abstracted away completely usually (but not always, especially for RF and microstrip design).
Decoupling capacitors for fast logic chips are understood to always be required on each supply pin, always as close to the chip as possible to reduce stray inductance (which dominates circuit behaviour at the speeds involved). Switching times in fast chips are on the timescale of a few nanoseconds.
